ELECTRONIC SUBMISSIONS:

Player Recognition orders may be submitted electronically by emailing this form to the Shrine Bowl at admin@shrine-bowl.com.  .  Digital pictures to be included in the recognition can be attached to the email.  Camera-Ready ads less than 5 MB can be submitted via email in jpeg or eps format and the copy typed in a WORD file, in the body of the email message or clearly printed on the application form.  All other formats will not be accepted.  If the total package exceeds 5 MB a CD will need to be provided. .
 

Photos should be 300 dpi at 4" x 6" and should be CMYK color and JPEG file format.
Ads should be EPS (Vector image) or high resolution PDF, TIFF, JPEG file format.

Payment must be received by check or money order before the recognition will be placed in the program
